What is Sub-Account form used for? How do we get it?

DOT sub-accounts are required when setting up employer clients for DOT lab testing.
These sub-accounts ensure that each client’s tests are properly tracked, billed, and reported under their unique company profile.

When you create an “Organization” in Lab Fusion, the system automatically generates a DOT sub-account for that client. This simplifies the setup process and ensures the account is properly linked to DOT-compliant testing procedures.

It’s important to enter accurate and complete information when creating the organization, as this data will be tied to all lab submissions and results for that client moving forward.

If you’re ever unsure whether a sub-account has been created properly, you can verify the setup within Lab Fusion or by contacting your lab representative.

For partner labs such as Quest Diagnostics, DOT sub-accounts must be manually added to ESP.
To do this, you’ll need to contact LABB directly and request that the sub-account be created and linked to your client within the ESP system.

This step is essential to ensure proper test routing, result reporting, and billing under the correct employer profile.

Be sure to provide LABB with all necessary client details when making your request to avoid delays in setup.
