Testing Through AIS (Electronic Authorization Form)

1. Greet and Verify

  • Greet the donor professionally.
  • Ask for the AIS Authorization Form (most donors will have it on their phone).
  • Request the donor’s photo ID and verify that the information on the form matches the ID.

2. Confirm Readiness

  • Ask the donor if they are ready to test or if they would like water before starting.
  • Offer water if needed before proceeding.

3. Move to the Lab Area

  • Escort the donor to the lab/testing area.
  • Ask to see their authorization form again and scan the barcode using your tablet through FormFox or eScreen.

Note: When using FormFox, always review the authorization form carefully — some clients may request a BAT (Breath Alcohol Test) in addition to or instead of a urine test.

  • If a BAT is required, you must complete it using a paper chain and send results to the client via email.

4. Begin the Collection

  • Once the barcode is scanned, the test will automatically open on your tablet.
  • Inform the donor that all personal belongings (phones, keys, wallets, etc.) must go into the locker before testing.
  • After placing belongings in the locker, have the donor wash their hands.
  • While they are washing, lock the locker for security.

5. Prepare for Specimen Collection

  • Retrieve a Quest DOT Split Collection Kit.
  • Hand the collection cup to the donor and explain the testing rules:
    • Do not flush the toilet.
    • Do not wash hands until returning the specimen.
    • The restroom door may be closed for privacy.

6. Receive and Process the Specimen

  • When the donor exits, put on gloves before accepting the specimen.
  • Check the temperature strip immediately.
  • If the temperature is in range, proceed to the next steps.
  • If out of temperature, follow DOT protocol (as outlined in the Testing Protocol Form).

7. Complete the Collection in FormFox / eScreen

  • Enter the temperature reading on the tablet.
  • Follow the prompts in FormFox or eScreen to complete the electronic chain of custody.
  • Apply the security seals to both specimen vials and scan each barcode into the system.
  • Have the donor sign and date the seals on the vials.

8. Print and Package Specimen

  • Once all electronic steps are completed, the system will prompt you to print three copies of the chain of custody:
    • Lab Copy – goes inside the specimen bag.
    • Collector Copy – keep for your records.
    • Donor Copy – provide to the donor.
  • Place the vials and lab copy into the Quest specimen bag and seal it securely.

9. Finalize and Release Donor

  • Unlock the locker so the donor can retrieve their belongings.
  • Return their ID and thank them for their cooperation.

If the donor asks about results, explain that results are sent directly to their company — Fastest Labs does not release AIS results to donors.
