Lab-Based Test (Omega Hair Test – Personal)


1. Greet and Identify Test Type

  • Greet the donor.
  • Ask what test type or panel they are requesting (e.g., hair drug test).
  • Once identified, look up the pricing for the Omega Hair Test panel.

2. Complete Authorization Form

  • Provide the donor with the Personal Release Form.
  • Instruct them to fill it out completely, including any email addresses they want results sent to.
  • Once complete, collect the form and ask for their photo ID to verify identity.

3. Create Sales Receipt and Take Payment

  • Open QuickBooks Online (QBO) and create a sales receipt for the test.
  • Enter all required information accurately.
  • Ask if payment will be cash or card:
    • If card, use the card reader.
    • If cash, select “cash” on the sales receipt.
  • Once payment is processed, print two receipts — one for the donor and one for your records.

4. Move to the Lab Area

  • Grab your tablet and an Omega Hair CCF.
  • Escort the donor to the lab/testing area..

5. Set Up the Test on Tablet

  • On your tablet, open your Walk-In Organization.
  • Select Universal Chain, then choose the Omega Hair Test panel.
  • Click “Start Test.”

6. Enter Donor Information

  • Scan the donor’s ID into the system.
  • Enter the donor’s phone number for MRO (Medical Review Officer) purposes.
  • Review the information for accuracy, then click Continue.
  • When prompted, verify the information and mark the checkbox to confirm.
  • Select “Collect Specimen.”

7. Prepare the Chain of Custody Form (CCF)

  • Fill out the top portion of the CCF:
    • Donor’s information
    • Reason for testing
    • The specific Omega Hair Test panel being conducted

8. Prepare for Hair Collection

  • Explain to the donor that the test requires a small sample of hair (approximately 120 strands) collected from the crown area or another non-visible section of the scalp.
  • Ensure the donor understands the process and is comfortable before proceeding.
  • Open a new Omega Hair Collection Kit.
  • Sanitize the scissors and collection clips using alcohol wipes.
  • Put on gloves and prepare the aluminum foil from the kit (shiny side up).

9. Collect the Hair Sample

  • Section the donor’s hair and isolate a small strand bundle.
  • Cut the hair as close to the scalp as possible — roots must remain intact.
  • Lay the sample roots facing out on the aluminum foil.
  • Fold the foil neatly to enclose the sample.
  • Place the folded foil inside the Omega collection envelope provided in the kit.
  • Have the donor sign and date the envelope where indicated.
  • Apply the security seal from the CCF to the sample envelope.

10. Link and Send the Specimen

  • On your tablet, click “Link Specimen.”
  • Scan the barcode from the Omega sample envelope.
  • Have the donor complete Step 5 (Donor Section) on the CCF:
    • Print full name
    • Sign name
    • Add today’s date
    • Include date of birth and phone number
  • Once the donor is finished, complete Step 4 (Collector Section):
    • Print and sign your name
    • Record the time and date of collection
  • On the tablet, scan the MRO copy of the CCF when prompted.
  • Click “Send to Lab.”
  • Place the sealed Omega envelope and corresponding paperwork into the Omega transport bag and seal it.

11. Finalize and Release Donor

  • Provide the Donor Copy of the CCF.
  • Return their ID.
  • Inform the donor that they are good to go and that results will be emailed to the addresses listed on the Personal Release Form.
