Are Authorization forms necessary with every customer, especially when they come in with DOT form on what to do?

Yes, authorization forms are absolutely required when testing a customer on behalf of a company. They ensure we can verify all necessary information for proper documentation. The company may send the form via email or provide a printed copy for the client to bring at the time of their appointment.

In other cases, such as walk-in appointments, the client will need to complete a personal authorization form upon arrival at your Fastest Labs location.

DOT testing follows a more detailed and regulated process. Unlike standard tests, the client does not sign an authorization form. Instead, they sign a section confirming that their test was not tampered with and that they observed the collection process without issue. Once the DOT documentation is complete and the client has provided all required information on the chain of custody form, you may then seal and submit the DOT paperwork.

Here are a few important things to keep in mind:

  1. Authorization Is Required:
    • An authorization form is required before testing any customer, except when conducting a DOT test.
  2. How the Authorization May Be Provided:
    • Email: The company may email the authorization form directly to your location.
    • Client-Delivered: The company may give the authorization form to the client to bring with them and present upon arrival.
    • Walk-In Customers: If the customer is a walk-in without prior paperwork, they will need to complete Fastest Labs’ internal authorization form before testing.
