 DNA Collection (Non-Legal – DDC)

1. Scheduling the Appointment

  • When a caller requests a non-legal DNA test, schedule an appointment based on the date and time they prefer.
  • Ask who will be participating in the test:
    • Mother
    • Child
    • Alleged Father
  • If all three are participating, provide the client with the price quote.
  • Confirm the appointment details before ending the call.

2. Check-In and Forms

  • When all participants arrive for their scheduled appointment, provide a Personal Authorization Form for the paying client to complete.
  • Make sure they list all email addresses where they want results sent.
  • On the back of the form, have them fill out the child’s information:
    • Full name
    • Date of Birth
    • Gender
      (This is required for the DDC submission.)

3. Payment

  • After the paperwork is completed, collect payment:
    • Log into QBO on your tablet.
    • Create a Sales Receipt.
    • Accept payment by cash or card.

4. Create Case in DDC Portal

  • Log into the DDC Portal.
  • Click “Submit Case.”
  • When asked if you need a Chain of Custody, select:
    • NO – because this is a non-legal test.
  • When prompted to choose a test type, select:
    • Standard Trio Paternity (for mother, child, and alleged father).
  • Enter the required donor information.
  • Click “Complete” at the bottom of the page.
  • Print the generated DDC Case Form.

5. Prepare the DNA Collection Kit

Use the Non-Legal DNA Kit (Red & White Envelope).
Inside you will find:

  • 3 small collection envelopes
  • 6 swab pouches (2 swabs per pouch = 12 total swabs)
  • Collection instructions & airbill
    • (You may discard the instructions and airbill — you won’t need them.)

6. Labeling Collection Envelopes

On each of the three small envelopes, fill out:

  • Donor’s name
  • Select which participant the envelope belongs to
  • Date and collection time

7. Collecting DNA Swabs

  • Put on gloves before handling any materials.
  • Each donor will be swabbed with 4 total swabs (2 pouches = 4 swabs).
  • Swab the inside of the donor’s cheek one swab at a time, ensuring full saturation.
  • Place each swab face-down in the correct small envelope.
  • Fold the envelope flap closed — you may tape it shut to ensure it stays sealed.
  • Repeat this process for each participant.

8. Packaging the Samples

  • Once all swabs are collected and sealed, place the three small envelopes inside the red and white DDC envelope.
  • Seal the large envelope securely.
  • Write the DDC Case Number clearly on the outside of the envelope.
  • The packet is now ready for shipment.

9. Sending Results to the Client

  • Monitor your DDC Portal for results.
  • Once results are posted, email them to the addresses listed on the Personal Authorization Form.
