DOT MIS Report Guide
What Is an MIS Report?
An MIS (Management Information System) Report is a detailed record of all drug and alcohol testing conducted for any DOT-regulated company. The MIS report includes data for urine analyses (UAs) and breath alcohol tests (BATs) across all testing categories, including:
- Pre-Employment
- Random
- Post-Accident
- Reasonable Suspicion
- Follow-Up
- Return-to-Duty
The report logs all negative and positive results, specifying the substances detected for positive tests. It also includes:
- Adulterated specimens
- Shy bladder with no medical explanation
- Refusals
- Canceled tests
For breath alcohol testing, the MIS report includes all testing reasons and the results (both positive and negative), as well as confirmation testing outcomes.
Most companies request an MIS report when:
- They are being audited by the DOT, or
- They are enrolled in a random testing program and require quarterly summaries.
How to Request an MIS Report
If your client is not enrolled with AIS but conducts testing through Labb, you can still provide them with an MIS report. There are two ways to obtain or create one:
Option 1: Request from Cynergy
If you have CCFs (Custody and Control Forms) that use your client’s Quest account number, you can contact Cynergy directly to request the MIS report.
Option 2: Create the MIS Report Manually
If you do not have access to the client’s Quest account, you can manually create the report using the DOT MIS form. Follow these steps:
Steps to Complete the DOT MIS Form
Section I – Company Information
- Enter the company’s details.
- Check the box for the DOT agency the company operates under (e.g., FMCSA, FAA, FRA, PHMSA, FTA, etc.).
Section II – Employee Count
- Enter the total number of covered employees.
- To calculate the average number of employees for the year:
- Review the employee list from each quarter.
- Add the total employees from all four quarters.
- Divide the total by 4 (for the four quarters).
- Example: (Q1 + Q2 + Q3 + Q4) ÷ 4 = Average number of employees.
- Example: (Q1 + Q2 + Q3 + Q4) ÷ 4 = Average number of employees.
- Review the employee list from each quarter.
Section III – Testing Data
- Enter the total number of tests conducted during the year.
Step 1: Gather Quarterly Data
- Go to the client’s organization in Labb.
- Use the date filter to isolate each quarter:
- Q1: 01/01/2025 – 03/31/2025
- Q2: 04/01/2025 – 06/30/2025
- Q3: 07/01/2025 – 09/30/2025
- Q4: 10/01/2025 – 12/31/2025
- Q1: 01/01/2025 – 03/31/2025
- After filtering, click “Accounting Export.”
- This download will include all tests completed for that quarter, along with the reason for testing.
Step 2: Tally the Tests
- Tally all UAs and BATs by reason for testing:
- Pre-Employment
- Random
- Post-Accident
- Reasonable Suspicion
- Follow-Up
- Return-to-Duty
- Pre-Employment
- Repeat this for all four quarters.
Step 3: Total the Annual Numbers
- Add up each testing category across all quarters.
Example:
- Q1: 10 Pre-Employment
- Q2: 7 Pre-Employment
- Q3: 5 Pre-Employment
- Q4: 10 Pre-Employment
- Total = 32 Pre-Employment tests for the year
- Q1: 10 Pre-Employment
- Repeat this process for all testing reasons.
Step 4: Complete Both UA and BAT Sections
- Enter all urine test data in Section III, items 1–13.
- Then complete the BAT (Breath Alcohol Test) section the same way.
Submitting the Report
Once all results are tallied and the form is completed, submit the finalized MIS report to your client.
This report will serve as the official record of all DOT-regulated testing conducted throughout the year.
