DOT Test Using an Authorization Form

1. Greet and Verify

  • Greet the donor professionally.
  • Ask for the authorization form and the donor’s photo ID.
  • Verify the name on the ID matches the authorization form.
  • If correct, grab your tablet and escort the donor to the lab.

2. Secure Personal Items

  • In the lab, instruct the donor to place all belongings (phone, keys, wallet, etc.) into the locker.
  • Once items are stored, have the donor wash their hands.
  • While they wash, lock the locker.

3. Open DOT Test on Tablet

  • On your tablet, select the organization.
  • Locate the DOT test form and click “Start Test.”
  • Scan the donor’s ID to prefill their information.
  • Select the correct DOT agency:
    • FMCSA, FAA, USCG, FTA, PHMSA
  • On the verification page, check all boxes confirming donor info is correct.
  • Click “Collect Specimen.”

4. Complete Step 1 on Quest DOT CCF

  • On the Quest DOT Chain of Custody Form (CCF), complete Step 1:
    • Donor ID information
    • First and last name
    • DOT agency
    • Reason for test
    • Collection site information

5. Provide Collection Cup

  • Retrieve a Quest DOT split collection kit.
  • Hand the donor the cup and explain the rules:
    • They may close the door for privacy.
    • Do not flush the toilet.
    • Bring the cup out immediately after collection.

6. Receive and Check Specimen

  • When the donor returns, put on gloves and accept the specimen.
  • Check temperature immediately.
  • If temperature is in range, proceed.
  • If out of temperature, follow DOT protocol using the Testing Protocol Form.

7. Transfer Specimen and Seal Vials

  • Transfer the specimen into the two split vials (A & B).
  • Apply the green security seals from the CCF onto both vials.
  • Have the donor initial and date the seals.
  • Mark the temperature on the CCF.

8. Link Specimen in Tablet

  • On your tablet, click “Link Specimen.”
  • Scan the barcode on each vial.

9. Donor Completes Step 5

  • Have the donor complete Copy 2, Step 5 on the CCF:
    • Print name
    • Sign name
    • Date
    • DOB and phone number

10. Collector Completes Step 4

  • After donor finishes, complete Copy 1, Step 4:
    • Print and sign your name
    • Record date and time
    • Select courier (FedEx)

11. Send to MRO and Lab

  • On the tablet, click “Send to MRO.”
  • Take a photo of Copy 2 and upload it.
  • Once uploaded, click “Send to Lab.”

12. Package Specimen

  • Place both sealed vials into the Quest specimen bag.
  • Insert Copy 1 (Lab Copy) of the CCF into the bag.
  • Seal the bag securely.

13. Finalize and Release Donor

  • Unlock the locker so the donor can retrieve belongings.
  • Return the donor’s ID and provide Copy 3 (Donor Copy) of the CCF.
  • Let the donor know they are good to go and results will be sent to their company.
