Lab-Based Test (Universal Chain – Quest UA, Personal)

1. Greet and Identify Test Type

  • Greet the donor professionally.
  • Ask what test panel they are requesting or if they have a Quest panel code.
  • Once identified, look up the pricing for the test panel.

2. Complete Authorization Form

  • Provide the donor with the Personal Release Form.
  • Instruct them to fill it out completely, including any email addresses they want results sent to.
  • Once complete, collect the form and ask for their photo ID to verify identity.

3. Create Sales Receipt and Take Payment

  • Open QuickBooks Online (QBO) and create a sales receipt for the test.
  • Input all required information accurately.
  • Ask the donor if they are paying by cash or card.
    • If card, use the card reader.
    • If cash, select “cash” on the sales receipt.
  • Once payment is taken, print two receipts — one for the client and one for your records.

4. Confirm Readiness

  • Ask the donor if they are ready to test or if they need water before starting.
  • Offer water if needed.

5. Move to the Lab Area

  • Grab your tablet and a Non-DOT Quest CCF form.
  • Escort the donor to the lab/testing area.
  • Instruct the donor to place all personal items (phone, keys, wallet, etc.) in the locker.
  • Allow the donor to wash their hands before testing.
  • While they do so, lock the locker and prepare the system on your tablet.

6. Set Up the Test on Tablet

  • On your tablet, open your Walk-In Organization.
  • Select Universal Chain and then choose the panel the donor will be testing for.
  • Click “Start Test.”

7. Enter Donor Information

  • Scan the donor’s ID into the system.
  • Enter the donor’s phone number for MRO (Medical Review Officer) purposes.
  • Review the information for accuracy, then click Continue.
  • When prompted, verify the donor’s information and mark the checkbox to confirm.
  • Select “Collect Specimen.”

8. Prepare the CCF

  • Fill out the top portion of the CCF:
    • Donor’s information
    • Reason for testing
    • The specific panel they are testing for

9. Collect the Specimen

  • Lock the locker once the donor is ready.
  • Retrieve a Quest single collection kit.
  • Hand the collection cup to the donor and clearly explain the rules:
    • Do not flush the toilet.
    • Do not wash hands until the sample is returned.
    • The restroom door may be closed for privacy.

10. Receive and Process Specimen

  • When the donor exits, put on gloves before taking the specimen.
  • Check the temperature immediately.
  • If the temperature is in range, fill the vial with the specimen and apply the seal from the CCF.
  • Mark the temperature on the CCF.
  • Have the donor sign and date across the seal.

 For out of temperature, insufficient sample, nothing provided, refusal, or non-negative results, refer to the Testing Protocol Form for appropriate procedures.

11. Link and Send the Specimen

  • On your tablet, click “Link Specimen.”
  • Scan the barcode on the vial.
  • Have the donor complete Step 5 on the CCF:
    • Print full name
    • Sign name
    • Add today’s date
    • Provide date of birth
    • Include phone number
  • Once the donor is finished, complete Step 4 (Collector Section):
    • Print your name
    • Sign your name
    • Record date and time
  • On the tablet, scan the MRO copy of the CCF when prompted.
  • Click “Send to Lab.”

12. Finalize and Release Donor

  • Provide the donor with the Donor Copy of the CCF.
  • Return their ID and unlock the locker so they can retrieve their belongings.
  • Inform them they are good to go and that results will be emailed to the addresses listed on their Personal Release Form.
