Are Authorization forms necessary with every customer, especially when they come in with DOT form on what to do?

Yes, correct, when a customer comes in for a company, we will always need an authorization form to be able to test them. The company may send it to you through email, they may give to client to hand to us upon arrival to our office and a walk in customer will fill out our personal authorization form.

But, for DOT tests, the donor must not be required to sign any type of indentification or release (i.e, the language we use for our rapid authorization forms).
