 Rapid Collection (Authorization Form)

1. Greet and Verify

  • Greet the donor professionally.
  • Ask for the Authorization Form and photo ID.
  • Verify that the name on the form matches the ID.

2. Confirm Readiness

  • Ask the donor if they are ready to test or if they would like a cup of water before beginning.

3. Move to the Lab Area

  • Escort the donor to the lab/testing area in a calm and professional manner.

4. Open the Test on the Tablet

  • On the tablet, search for the organization listed on the authorization form.
  • Select the appropriate rapid test cup and test panel for the donor.

5. Secure Personal Items

  • Instruct the donor to place all personal items (phone, keys, wallet, pens, etc.) into a locker.
  • While the donor is placing their items away, scan the donor’s ID.
  • Once done, lock the locker for security.

6. Hand Hygiene and Consent

  • Have the donor wash their hands before testing.
  • Once they return, have them sign the tablet and sign the authorization form.
  • Confirm that signing means they agree to participate in testing today.

7. Explain the Collection Rules

  • Show the donor the rapid test cup and specify that at least 30 mL of specimen is required.
  • Keep the lid with you at all times.
  • Clearly explain:
    • Do not flush the toilet.
    • Do not wash hands until the sample is returned.
    • The door may be closed for privacy.

8. Receive and Check Specimen

  • Put on gloves and accept the specimen from the donor.
  • Check the temperature strip immediately.
  • If temperature is in range, secure the lid tightly.
  • If out of temperature, follow client protocol.

 For out of temperature, insufficient sample, nothing provided, refusal, or non-negative results, refer to the Testing Protocol Form for step-by-step instructions and documentation requirements.

9. Scan and Record Results

  • Place the sealed cup on the testing stand.
  • Scan the cup using your tablet and ensure the tablet screen faces away from the donor.
  • If the result is negative, select “Temp In Range” then “Sign & Send” to upload the results.
  • If non-negative, follow the confirmation/send-out process listed in the Testing Protocol Form.

The donor must remain present until the collection and data entry are fully completed.

10. Finalize and Release Donor

  • Discard the used test cup properly.
  • Unlock the locker so the donor can retrieve their belongings.
  • Return their ID and thank them for their cooperation.

If the donor asks for results, explain that you are not authorized to share them — results are sent directly to the company.
