 DOT Test (Labb Passport)

 Greet and Verify

  • Greet the donor professionally.
  • Ask for the Labb Passport (usually shown on their phone).
  • Check the TPA information on the passport — it should list your Fastest Labs location.
  • Ask for the donor’s photo ID and verify that the information matches the Labb Passport.

2. Move to the Lab Are

  • Grab your tablet and escort the donor to the lab/testing area.
  • Have the donor empty their pockets and place all personal items (phone, keys, wallet, etc.) into the locker.
  • Ask the donor to wash their hands before beginning the test.

3. Scan and Verify Information

  • Scan the barcode on the Labb Passport using your tablet.
  • Review the first page and verify the donor’s information.
  • If all information is correct, check “Select All” to confirm.

4. Prepare for Collection

  • Retrieve a Quest DOT Split Collection Kit.
  • Fill out Step 1 on a Quest DOT CCF (Chain of Custody Form) with the donor’s information and testing details.
  • On the tablet, click “Collect Specimen.”
  • Lock the locker once the donor is ready.
  • Hand the collection cup to the donor and clearly explain the testing rules:
    • Do not flush the toilet.
    • Do not wash hands until returning the specimen.
    • The restroom door may be closed for privacy.

5. Receive and Process the Specimen

  • When the donor returns, put on gloves before taking the cup.
  • Check the temperature strip immediately.
  • If temperature is in range, proceed with the next steps.
  • If out of range, follow DOT protocol as outlined in the Testing Protocol Form.

6. Transfer the Specimen

  • Pour the specimen from the collection cup into the two vials (A & B).
  • Allow the donor to wash their hands after the transfer.
  • Place the security seals from the CCF onto the vials.
  • Have the donor sign and date across both seals.
  • On the CCF, mark the temperature reading in the collector section.

7. Link and Complete in Tablet

  • On your tablet, click “Link Specimen.”
  • Scan the barcode on each vial.
  • Have the donor complete Copy 2, Step 5 on the CCF:
    • Print name
    • Sign and date
    • Include date of birth and phone number
  • Once the donor is finished, complete Copy 1, Step 4 (Collector Section):
    • Print your name
    • Sign
    • Record time and date of collection

8. Send to MRO and Lab

  • On your tablet, click “Send to MRO.”
  • Take a photo of Copy 2 of the CCF and upload it when prompted.
  • Once uploaded, click “Send to Lab.”
  • Place both sealed vials into the Quest specimen bag along with Copy 1 (Lab Copy) of the CCF.
  • Seal the bag securely.

9. Finalize and Release Donor

  • Unlock the locker so the donor can retrieve their belongings.
  • Return the donor’s ID and provide them with Copy 3 (Donor Copy) of the CCF.
  • Inform the donor that they are good to go and that results will be sent directly to their company.
